New Patient Registration

It is the Elms Medical Centre policy not to accept out of area patients onto to our practice list. All patients must be living within the practice boundary to be accepted as a patient.

All new patients will be offered a new patients health check prior to registering. 

Please complete the new patient registration forms on the above “Registering Online” tab.  Alternatively the form can be collected from our reception.

It is essential that you bring your current repeat medication form with you when registering to ensure there are no gaps in reissuing medication.

You will also need to bring with you proof of address and photo ID, ‘If you are unable to provide these documents please talk to our reception staff’.

New patient registrations can be completed at the practice after 4pm each day
